The drivers side lip of the reservoir seal falls into the reservoir...despite being carefully installed. It's happened 3 times...as if there is a vacuum pulling it in.

Any ideas??

When this happens, does brake fluid squirt out from under the seal when applying the brakes?

After a 50 minute commute home, pulling into the driveway, I had a brake light. Every time I applied the brake, fluid would shoot out from under the reservoir lid. I reseated the lid and no leaks now. After reading your post, I wish I would have paid more attention to the seals position.

I would fully depressurize the system (per the instructions imprinted on the lid), check the fluid levels (filling if necessary), and then turn the car on to watch the system behavior. When the pump kicks on it will suck fluid out of the _passenger's_ side, but it might yank on things enough to draw the seal in over on the other side.

I would expect this more in the passenger's side, since that is the side used to pressurize the assist mechanism. I could see it sucking the seal in if you didn't have sufficient fluid. The driver's side doesn't seem to make sense. Make extra sure the seal is fitted properly into the resivoir cap and the assembly is fully snapped down all around.
